Total Personal Property Tax

Go to Page 1, Line 10 on your 2004 Personal Property Tax Form 920.

Listed Value of Machinery & Equipment

Go to the bottom of Page 2, on your 2004 Personal Property Tax Form 920 in Schedule 4 under the "Listed Value" column.

Total all listed values for items described as machinery & equipment and report the total here. Please exclude any furniture, fixtures, or supplies listed.

6. Listed Value of Furniture & Fixtures

Go to the bottom of Page 2, on your 2004 Personal Property Tax Form 920 in Schedule 4 under the "Listed Value" column.

Total all listed values for items described as furniture & fixtures and report the total here. Please exclude any machinery, equipment, or supplies listed.

10% Rollback

On your 2004 Real Estate Tax bill find the amount next to "RB" under "Full Year Current Taxes". If you do not have a bill or cannot find the rollback amount, estimate by taking your total real estate taxes paid times 11.11%.

8.

Adjusted Total Receipts

Go to Page 1, Line 1A from your 2003 Federal Tax Return. Subtract non-ohio sales and dealer to dealer transactions from this number.

9.

Franchise Tax

If you are a C-Corp list the total tax paid on your 2004 Form FT-1120 on Page 1, Line 20.
